INITIAL CONSULTATIONS
Ahead of all initial consultations, we will ask you to answer some Initial Client Intake questions either via our booking system when making your appointment through our website, or via email. This ensures we are on the same page for the first consultation and can make the best use of time. The form asks for details such as your name, contact details, date of birth, allergies and diagnosed medical conditions. It also asks you to briefly summarise why you want to see a Naturopath and what you hope to get from the first consultation. This information is important for us to plan the consultation, make the best use of the time we have together and ensure relevant information is to hand. Please ensure that you sign the bottom of the second page

Peter Balogh Naturopath Clinic has a 0-tolerance policy that does not tolerate inappropriate behaviour, aggressive comments or gestures. If this occurs, the practitioner reserves the right to terminate the consultation immediately and the client will incur 100% of the fee
